Mental illnesses are among the most common health conditions in the United States. More than 1 in 5 US adults live with a mental illness. Over 1 in 5 youth (ages 13-18) either currently or at some point during their life, have had a seriously debilitating mental illness.
They range from common problems, such as depression and anxiety, to rarer problems such as schizophrenia and bipolar disorder.

discriminatory action in sufficient detail to inform the Assistant Secretary for Civil Rights (ASCR) about the nature
and date of an alleged civil rights violation. The completed AD-3027 form or letter must be submitted to USDA by: 1. mail: U.S.
