Home Health Services:
Skilled nurse, bath aid, physical, occupational and speech therapy.
Office hours for skilled nurse/ bath aid are:
Monday- Thursday 8am-5pm

Admission criteria can only be made under the direction of a physician or other allowed practitioner based upon your care needs and home bound status.
Our Services Include:
Skilled nursing works with physician to update your plan of care. Evaluation of patient needs; performance of skilled nursing procedures;
Education of patient, family members and caregivers on disease proceeded; self-care techniques and prevention strategies: coordination of patient care and services with your physician and other health care team members.
Physical Therapy/ Occupational Therapy / Speech Therapy provided by licensed therapist or therapy assistant. Your therapist will provide specific information about the services and treatment you will receive. Medically necessary therapy serviced must be coordinated with one agency while you are receiving medicine covered home health services.
Home Health Aid:
Services delivered under the licensed supervision of a registered nurse. Typical duties include bathing, shampooing hair, changing bed linens and assisting with other activities of daily living.
We do offer private pay for these entities
